I changed settings to show only the first name of contacts. (Settings > Contacts > Short Name > First Name Only).
However in Email and Messages, the full names always appear. When "Prefer Nicknames" is activated, nicknames do appear. But generally both first and last names appear regardless.
Device is up to date (iOS 12.1.2) but this has gone on a while.
Suggestions appreciated.

Do you have contacts enabled in any of the 3rd party emails you added?
When you go to settings>Apple ID>iCloud - are contacts turned on?
If you go to settings>contacts - what is set as the default?
Sort Order: First, Last
Display Order: First, Last
Short Name: (see above)

Absolutely yes and I can see & update contacts from multiple devices.
But it seems to me either (a) this is a bug and iOS does not do what it would seem to say it can do or (b) if the location of the contacts matters -- device vs iCloud -- then it's a poor user experience.
This may be the case, but it shouldn't matter.
If there are a lot of strings attached to using this feature, then when I try to use it the experience should be "you can't do this because...".
Another way in which Apple has stupid crap like animated emojis but can't get the basics right.
So thank you for your help, but I have concluded this is a bug and have reported it as such.
I don't know what the defaults are.
Presently they are:
Short Name ON
First Name Only SELECTED
Prefer Nicknames ON
