Why is a Progressive Web App (PWA) not downloading in iOS properly?

I am receiving an error when I open new Safari window, download a PWA (by going to "Add To Home Screen"), and it does not show up as an app icon within my home screen. This PWA does show up in spotlight when I search for it, but when I try to open the PWA in the search results, it opens up Safari and goes to the website of the PWA rather than the PWA. I was able to reproduce when on apple.com, google.com, and other PWA-ready sites.
