cant use camera for mobile deposit in banking app. says doesn't have permission

I have an iphone 6, which was replaced under warranty a month ago. Since that time, I have been unable to make mobile deposits on my banking app. It says "Error - the application doesn't have permission to use camera device, please change privacy settings." I have done all the necessary things to correct this such as checking settings to make sure it is enabled for photos, etc. Have even reset the phone. Nothing corrects. Any suggestions? I am beginning to think there is a glich with ios 12.1.4
