Not able to do hardware test in my macbook pro mid 2012
after connecting to the WiFi it displays this error
What could be the issue?
Also my macbook suddenly freezes in between and hangs for 10 min or so.
Please advice.

If this is a non-Retina model, then you could have a failing hard drive or a bad hard drive cable. To check the health of the hard drive, create a bootable Knoppix Linux USB drive using Etcher. With the Knoppix USB drive connected, power on the laptop and hold the Option key to get the Apple boot picker menu and select the orange icon labeled "EFI". Once you reach the Knoppix desktop, click on the "Start" menu in the lower left on the Taskbar and navigate to "System Tools --> GSmartControl". Select your drive in GSmartControl and post a picture of all the "SMART Attributes" and note if any errors are listed in the "Error Log" tab of GSMartControl.
What macOS are you currently running?
That looks like a efi firmware problem.
If option command D does not bring it up, then you are out of luck.

I have tried with connecting an SSD and with the on board HDD, in both cases i am getting the same error while doing hardware test with Option+D.
Not sure if this is a motherboard issue, but the mac runs smoothly without any lag yesterday and today again it freezes multiple time on start up itself. The keyboard also not working and I was not able to pass my password. I had to forcefully keep on pressing the power button to turn off the mac.

What happens if you remove the Hard Drive/SSD and try booting the Apple Diagnostic?
Try an SMC reset & PRAM reset.
Trying to boot the Knoppix USB drive does several things at once. One it will test to see if the system can boot from a local drive bypassing the internal drive and any network issues involved in the other attempts. If it boots, then it allows you to check the health of the hard drive.

In or out of warranty you can get a free over the counter 'Apple Service Diagnostics' test /assessment.
Let Apple tell you what is wrong, and go from there.
