Child's Apple ID purchase could not be completed – how to resolve?

I've approved my child's Apple ID purchase on his iPad, entered my approval and password on my computer when prompted, all looks good, but my child's iPad says the "purchase could not be completed." What to do? There is no option to approve in person...

Contact iTunes Support. Either use the form or scroll to the bottom of the page in the 2nd link. Follow through the screens to get a chat or telephone option.
