Inconsistent New Podcast Notifications

Hello - I manage a podcast on iTunes and I notice that sometimes I'll receive a push notification when the new episode goes live and sometimes I don't. I'm confused why I wouldn't consistently get a notification each time a new episode launches. Any help/ideas would be greatly appreciated.

No notifications in Apple Podcasts Even though I've allowed notifications for Apple Podcasts and selected podcasts I follow I never seem to receive a notification for a new episode. This applies to both macOS and iOS. I've also noticed that there is no longer an option to select refresh rate, so I'm not sure when I should expect such notifications.

I accidentally dismissed the notification request from Podcasts app and it's not showing up in the system settings now I accidentally dismissed the notification request from the built-in Podcasts app and it's not showing up in the notification setting area. Is there a way to reset the notification settings or let Podcasts app resent the request?

Podcast Not Appearing on Feed Hello my podcast 'The Boro Breakdown Podcast' isn't appearing on our profile feed? When searching for episodes in the search bar they appear, I.E. No Place Like Home Boro Breakdown. However, our actual profile is not updating and hasn't updated for the last 2 episodes. Can anyone help?
