Incorrect covers showing for albums

For as long as I’ve had iTunes I’ve had a weird bug where incorrect covers will show for albums, if they show at all, after I’ve synced them to my phone even though it is correct in iTunes. Also, albums in my library will randomly split themselves into multiple albums even though going into the information it’s all exactly the same and they were even ripped from the same CD which I have the physical, legal copy of.
