I have been using safari recommend passcodes for sometime now but all of a sudden, I cannot force safari to recommend a secure passcode on a majority of the sites I use. Nothing has changed since the last passcode safari recommended. Any ideas 💡 I even tried creating new accounts on the most common sites I know of and safari will not recommend a new password. For example, macrumors, Gmail, yahoo and a few others.

Clearing website data and cookies did it. My suggested passwords and keychain logo now appears. Do you have an idea of what would of caused the issue.

Try setting up another admin user account to see if the same problem continues. Please post back on whether or not this worked. Also try the Safe Mode. Please post back on whether or not this worked.
Isolating an issue by using another user account
If it works in the Safe Mode, try running this program when booted normally and then copy and paste the output in a reply. The program was created by Etresoft, a frequent contributor. Please use copy and paste as screen shots can be hard to read. Click “Share Report” button in the toolbar, select “Copy to Clipboard” and then paste into a reply. This will show what is running on your computer. No personal information is shown. If the log won’t post, try posting it in Pastebin and provide a link in a reply Pastebin
Try double clicking the Home button and swipe Safari upwards. Go to Settings/Safari and clear History and Website Data. Open Safari and test.

You are welcome.I don't have a technical explanation, but sometimes it is necessary to clear out all the built up caches and history to get things working normally again.

I should of stated, it is an iOS issue. My Mac system does recommend passcodes and my keychain operates fine.
My iPad Pro and iPhone XR are not. The key does not appear except for recommending accounts I have already established.
