Mac partition disappeared after OS X reinstall on Boot Camp setup

After some problems with my High Sierra partition, I decided to erase the partition and install Mojave from Internet Recovery. This all appeared to work fine (my Windows Boot Camp partition remained intact) and I booted into OS X several times subsequently. However, I then booted into Windows via ALT at startup and since exiting Windows, I haven't been able to boot back into Mojave. The boot menu (via ALT) now only shows the Windows partition.

I again booted into Internet Recovery and ran Disk Utility. My OS X partition seems to sort of be there but Macintosh HD is not listed on the left:

I also tried clicking "First Aid" which said it was successful but made no difference.

I then went into Terminal and entered:

diskutil list

diskutil cs list

This was the result:

Any ideas how I can fix this?

Thanks.

Your Internet Recovery OS seems to be older than Mojave, which can cause problems. I do not recommend any manipulation of the internal disk, any further, using Internet Recovery.

If you have a 32+GB external disk (USB3/USBc/TB is preferred), re-install Mojave on the external disk, upgrade the external disk to the latest Mojave release, and boot from it. See How to reinstall macOS from macOS Recovery - Apple Support for reference.

Once you have booted from it, post the output of

diskutil list

diskutil apfs list

You may also want to temporarily disable SIP using csrutil, if your GPT needs to be manipulated to correct any issues.
