What happened prior to this discovery? Try Empty/corrupt iTunes library after upgrade/crash - Apple Community.

Do you have a subscription to either Apple Music or iTunes Match? Are these items stored locally or in the cloud?

Ideally you would have restored the library from backup as soon as the issue became apparent. Perhaps there is something odd going on where an incomplete list gets to the device and then that version syncs back to the library. It may help to see this post on erratic syncing and reset syncing between the library and the device.

They are NOT apple downloads they are 20 year old Napster mp3’s

It has been that way for over 6 months. songs are greyed out on playlists on the phone. When I go to my imac, the play lists were empty. The songs were still in my library but many of the playlists lost their content
no events or changes made to computer of any significance

It seems this is not a syncing problem because several old playlists on my iMac have become empty resulting in the music disappearing from my mobile devices. Strangely, the music titles in the playlists on the mobile devices are in the playlists, but they are either grayed out or when they look normal and I try to play them I get a notice that this music can not be played in this region of the world. The title then becomes grayed out.. this is not recently downloaded music but legacy music I have had for in my library for over 15 years. Almost seems like apple is purging music (mp3) that I did not pay for years ago ( Napster)

I don’t know what iTunes Match is. I don’t use Apple Music. Only occasionally buy individual music pieces I don’t store the music in the cloud ( as far as I know )
That's very odd then. iTunes shouldn't have an issue with generic mp3 files, regardless of the source.

The "cannot be played in this region" tracks. I assume they are iTunes Store purchases then. I think this may be an alternative to the persistent authorization request that sometimes comes with corrupt downloads from the store. Removing your current copy and redownloading may help.
