Siri now confusing ‘Reminders’ with ‘Notes’ ??

Siri would recently understand “put [something] on my grocery list”, correctly, and she would add that item to my “Grocery List” in Reminders.

However recently, she has started creating a “note“ with the name of the item I’m trying to add to my grocery list. ????

Anyone have any idea why this is happening? Or having the same issue?

Follow up: Prviously I was to be able to add the first item by saying “hey Siri, add [eggs] to my grocery list, and after Siri confirmation I could immediately press the Siri button on the screen and say “add turkey bacon” and turkey bacon would be added to my grocery list as well. Now when I press the Siri button to add a second item the second item is created in notes. ?????

Could this possibly be glitches in the upcoming redesign that had just leaked into the current iOS?
