iPhone Disabled Issue

My sons iPhone keeps being disabled every time he enters the correct passcode, it is correct, he has changed it. Weirdly there is also a slightly different looking passcode box which is different to where it is usually entered. Not sure what to do! Any ideas? Picture below

I have forgotten my passcode but the phone comes up with Emergency and a telephone keyboard. I cannot get

to the passcode.
