After update to v13.0 (now 13.1.2) ringer volume continues to reset to low. Have the "change with buttons" set to off. Will set the ringer to max, later will check (after missed calls and texts) and ringer is set to low or off. Have performed a hard reset several times as well.
Any suggestions?

Check all these 3 settings (especially #3 a & b)
1. Silent switch (above volume up button) isn't in silent mode.
2. Settings → Do Not Disturb → Do Not Disturb = OFF
3. Settings → Sound & Haptics → RINGER AND ALERTS:
a. Slide Volume all the way to the right.
b. Change with Buttons = OFF
Hmm... Although this problem started with iOS 13 versions, it only seems to occur when Keyboard Clicks are enabled / turned on AND my Bluetooth headset/earpiece is connected.
After a couple of short tests, the issues appears to go away when Keyboard Clicks are disabled / turned off. This is with my Bluetooth headset attached and typing in notes. Turn Keyboard Clicks back on and the problem immediately returns.
I drive a 2019 Chevy. I would think BT 5 would have been implemented since it was released in 2016/2017.
But what do I know.
Apple support has really surprised me on their lack of interest on this issue.
Just as an update - after installing iOS 13.1.3, volume still resets to low.
Just today, have manually increased volume three times to max.
