If I was to buy an apple watch in the UK, would I be able to use it in Spain if I put a spanish sim in my iPhone and vice versa each time I travel back and forth? Also if I bought the cellular version would I have to paybthe monthly tariff or can I use the watch using my iPhone like the gps version?

A GPS only Apple Watch just depends on the iPhone for radio services, and works anywhere the iPhone works. A GPS Apple Watch can do Everything a cellular model can do as long as the iPhone is nearby.
