If an apple watch is purchased from an ECG approved country ( USA ) and activated later at another ECG approved country ( India ) will the feature work without any problems ? Also will the feature work in other approved countries when I travel ?

No, the feature (ECG) will only function in the country where it is approved for use and where it was purchased. In other words purchase it in the USA ...you can only use that function in the USA.
