How do I find and install index.xlm file

Documents created by Pages '08 and '09 contain an index.xml file. Documents created by Pages v5 — v8.2 are in an incompatible Pages document format that does not use the old index.xml file. So the older Pages application throws the dialog about the missing index.xml file.


Solution: Formally Quit Pages '09 (not the red traffic light on its document). Then right-click on the newer Pages document, and choose Open With > Pages (default) (n.n). This will open the document in the proper Pages application. If you do not have a newer Pages installed on your Mac, you will need to open this newer document in Pages for iCloud.
