Uninstall CleanMyMac by following the developer's instructions since AV & cleaning apps are not needed on a Mac. These type of apps usually cause more problems than they solve.

It took a while to run EtreCheck which is most likely why it is reporting a drive failure. To check the health of the hard drive run DriveDX. If the drive shows as healthy, then post the DriveDX report here using the "Additional Text" icon which looks like a piece of paper.

You are running low on memory. Replacing one of the 4GB modules with an 8GB module would give you a total of 12GB of memory. It is best to purchase memory from Crucial or OWC using the tools on their respective websites to identify the correct memory to purchase.

Even if the hard drive is healthy you could still get a good performance boost by upgrading to an SSD.

Your hard drive is definitely failing. Your read and write speeds are practically zero. Your filesystem test failed to complete. The most important thing to be done at this point is to make sure you have a good backup. Unfortunately, EtreCheck needs Full Drive Access to check the status of your Time Machine backup. If you don’t already have a Time Machine backup, you need to make one. Do this now.

I don’t know if you can take your machine to an Apple store or not. Apple may consider it too old to repair. You should be able to find an Authorized Apple Service Provider, or even an independent repair shop, that will replace your hard drive with an SSD. This is a very easy repair for this machine. It shouldn’t take any longer than 15 minutes. Make sure to get your hard drive cable replaced as well. And as long as you are opening up the machine, may as well upgrade the RAM too.

You should probably also consider a new machine. Older machines are subject to failure in other ways than just the hard drive. You could spend a couple hundred dollars on this machine and see it fail a month later.
