HP printers printing "data page"

I have a 2018 MacBook Pro running Mojave. Since I got it a few months ago, both of my HP printers are printing a "data page" (shows file name, directory, template, etc.) for each document I print. If I print multiple copies of the same document, it prints the data page for every copy. I've looked in system settings and can't find a way to address this. How do I turn off this feature? It's driving me crazy.

sorry, the terminology changes in different parts of the System, Banner page is what CUPS, the MacOS prints spooler calls it.


Most Printer Drivers and dialogs call it "Cover Page" and you will see it among the drop down boxes form which to choose on the Print Dialog.



The word Preview, half way down the right side is a set of drop-down menus that controls the content of the bottom right quadrant of the Print pane. one choice is often "Cover Page".
