why ECG app not showing in my apple watch which i bought from UAE, as people says in uae we can't use ECG app and my question is if change my app store region to US can i use ECG app?

Taking an ECG with the ECG app on Apple Watch Series 4 or later - https://support.apple.com/HT208955 - "Make sure that the ECG app is available in your country or region and that you're in the country or region where your device was purchased." So unless it is supported both in the country of purchase and where you reside, it won't work.

The Apple Watch knows what country it was manufactured for, and has GPS so it knows where you are.
As Limnos says, when both country of purchase and country where used governments approve, ECG will work on your Apple Watch.
If I change my App Store region to US can I use ECG app?
