Pdf hand written notes

I use a PDF that I open on my iPad, I write the information I need and save it then email, next client I use same pdf, I erase all notes and fill out new info save and email it, every time I erased I thought is was going back to original however it is not it is clean for me to start again however it is loading more MB, this particular form was once 4.4Kb it is now at 34.6MB meaning - I can’t use it on email.

I lost the original pages/word form (before was saved into pdf)

question. Is there any way I can go to this pdf erase all hand written notes and Data to revert it to original so I can “start again”?
