Unlike LibreOffice Writer, or MS Word, Pages lacks an interactive form designer, and does not support any form creation for electronic document entry.

You can however, enter checkboxes from the character palette (control+command+Spacebar), search for ballot, and then double-click the character to insert ☐ into your Pages document. This might be useful for printed paper forms.

Do you want checkboxes that can be checked or unchecked by clicking the box, or do you want open boxes that can be printed, then checked (or not) using a pencil?

Here are two examples of the second type:

The six item checklist at the top is a Bulleted list, with Image bullets.

It is created using the Bullets & Lists settings near the bottom of the Text Format Inspector (see image below).

The box image used is in the bottom section of the Current Image choices in my (likely older) version of Pages.

The second example is a box character selected from emoji and symbols and listed in Keyboard preferences > Text as the text to substitute for the string, '▢'. Typing ▢ followed by a space replaces the ▢ with a checkbox. the tab shown in the second example allows the user to cancel the substitution and leave the typed text as is.

If you need 'live' checkboxes in your document, you will need to use a different application, and Pages does not support 'active' checkboxes in regular text or in tables.

Barry
