How about

Third party? SafeBoot https://support.apple.com/en-us/HT201262
To get a good look at your System config. for conflicts or issues, you can run this trusted utility http://etrecheck.com
Try dragging it off the menubar, or command drag it off.
Control click may give you some insight(?)

I've tried this as well. The icon didn't go away even after unchecking all the apps using the microphone.
Dragging the icon anywhere outside the command bar will cause a stop icon / not allowed icon to appear beside it and then it runs back to the command bar.
Hi Leroy,
My brother eventually helped figure out where the icon came from. It was from google meet / hangout. I had a call some time ago. Once I force quit the browser (Safari) it simply went away.
Thank you for all your help. It wasn't a bug after all. :-)

Good computing AyowoleT. Glad to hear you solved the problem.
I can drag it but where do I drag it to as it keeps going back?
Thank you.
Gone through all the options except the last one, nothing changed.
Do I run etrechecker when I safeboot or will it work when I just run it normally?
