Yes. however this is not the case as I have explained repeatedy. If I make a call it should appear in my recents list. If a call comes in and I do not answer it it should appear in my recents list, whether they leave a voicemail or not. If a call comes in and I answer it it should appear in my recents list. None of this is happening.

What is the exact problem you have; it may or may not be the same problem. Have you tried opening Recents and tapping on “All” rather than “Missed”?
I was having the same thing happen. Some of my recent calls would show up in the recents list but most wouldn't. I resolved it (at least for now) by going to: Settings/General/Reset and doing Reset Network Settings. After the reset, the missing calls appeared.
Of course I have. Whenever a call comes in or I make an outbound call it should appear in the "recents" list. They do not. It's intermittent as well. Sometimes they DO appear. And no I do NOT make so many calls that they are falling off the bottom of the list.
BTW I am a software engineer with over 30 years of experience. I think I know when something is not working as it should. I am mererly trying to ascertain if this is an iOS bug or something changed with the way iPhones work since I had my first one, a 3gs.
It's recent entries that are not showing, not oldest. Call forwarding is not enabled. All activity is on the phone directly. And yes "All" is selected. And missed calls that go to voicemail used to appear in the recent's list.
Whether anyone else has the same issue is irrelevant; you have the issue. With over 1 billion phones in use no matter what problem you have it is pretty much guaranteed that there are others who have the same or a similar issue. Knowing that doesn’t help you solve the problem.
Some characteristics of the Recents list:
Beyond these, every outgoing call should appear, and every call that causes your phone to ring should be on it. If that isn’t happening you need do some troubleshooting. As a start, are you sure that you tapped “All” and not “missed” at the top of the Recents list?
No one has access to my phone except me. I use Face ID to unlock my phone. Besides it's more than one call. It's almost all calls I make or receive.
I have 256 mb of storage of which about 20% is used. Anyway it might be because I switched carriers recently. Not that big a deal just wanted to know if others were experiencing the same issue.
Its not a problem I’ve seen posted before, so it is almost certainly an issue with your phone. About the only Recents problem that gets posted regularly is that people don’t like the fact that the list has only 100 entries.
You or someone with access to your iPhone may have deleted it.
I have the same problem on my 11 pro max. The problem has been an issue since I received it. Sprint is my carrier.
Calls that go to voicemail will only appear on the phone if they actually get to the phone, which is why I mentioned ring. If the phone is off, or doesn’t have a network connection when a call arrives, or the tower you are connected to doesn’t have any available channels, it will go to voicemail on the carrier’s voicemail server. As the phone never sees it, that call will not be in Recents.
This seems to have fixed it. I did change carriers and swapped sim cards so it makes sense that this was the issue.
Do you have available storage? The phone should have at least 3 GB available at all times.
Beyond that, all I can suggest is restore the phone to factory settings, then restore your backup.
