Blue Box around screen and/or Text Field

Hi,

I’ve enclosed a photo of a blue box that randomly appears usually in conjunction with pressing the Space Bar on an external keyboard. It happened after the last iOS upgrade and thought it was associated with Accessibility, but even after turn off all the Accessibility features it still randomly appears from time to time. Any suggestions on what could be the cause of the mysterious Blue Box? Thanks in advance for your suggestions and help.
