Catalina 10.15.4 my AOL images won't download

Verify your Mail>Preferences>Viewing> check remote images. Reboot if changes are made.

If no resolve reinstall the combo update on top of your existing macOS 10.15.4:

Combo update macOS 10.15.4 https://support.apple.com/kb/DL2037?locale=en_US

and test.

If this is browser mail—than Mail.app preferences is irrelevant.

Test a different browser.

To trouble shoot further you can:

—  Try a SafeBoot https://support.apple.com/en-us/HT201262

  Takes noticeable longer to get to the login screen, does a 5-15 minute disk repair before it fully boots up, and certain system caches get cleared and rebuilt, including dynamic loader cache,  etc.  Login and test. Reboot and test as caches get rebuilt.

It is normal for Safe mode third party system modifications and system accelerations are disabled, as you test, remove malware etc. reboot back to normal mode.

This test will tell you if third party interference; extensions etc are not loaded in safe boot mode.

  This will tell you if it a universal issue or isolated to your user/admin account.

I've gone into Safari/Preferences/Website/General/Downloads and 'mail.aol.com' has it as 'Allow'.

Apple/Prefernces/Internet Accounts. AOL is listed, checked off and seems good.

What are you saying here— you use browser mail, and using Safari you can not see image in your browser mail?

Try a different browser to test.

FWIW, I happen to still have an active AOL account (don't know why), I tested mail.aol.com in Safari on Catalina and attachments download when I click them. Sorry, I don't know why yours aren't.
