First, open Disk Utility and make sure that the disk is working correctly. You can do this by running First Aid or typing:
diskutil repairVolume diskIdentifier
Try creating the installer after that. Also make sure that the disk is formatted as HFS (macOS Extended) so that the bootable installer can be created.
