Usually I/O Errors indicate a hardware failure. I rarely use "diskutil" to erase drives/volumes so I'm not certain if you are using it correctly here especially if part of the partition structure has changed or been damaged after a failed attempt. What happen if you use the Disk Utility GUI to erase the whole physical drive as GUID partition and MacOS Extended (Journaled) assuming you are using macOS 10.11+, otherwise if you are booted to macOS 10.6 to 10.10 you need to partition and format the whole physical drive using these instructions:

Sometimes if you receive an error using the Disk Utility GUI app you need to do the exact same erase or partition/format again immediately after the error since I've seen Disk Utility fail on the first attempt for some unknown reason, yet succeed on the second attempt made immediately after the first failure.

You can try running the Apple Diagnostics to see if any hardware issues are detected, but the diagnostics do not detect many types of drive failures. I can provide instructions for creating and using a bootable Linux USB stick to check the health of the drive if you are interested if the diagnostics do not report any issues.

Try Mounting it on your Own:

If your hard drive is not registering, then you may need to mount it. Do so in Recovery Rode. Use Disk Utilities to mount the hard drive. Go Here: About macOS Recovery - Apple Support.

As it Reads: Could not mount disk@s2 with name MacintoshHD after erase
