I have an iPad 5th genaration running iOS 13.4.1, set for Italy.

On writing a text, when changing the character set (bottom left key "123" / "#+=", the image on two keys doesn't change and keep showing both alternatives "!" with "," and "?" with "." (all the other keys change the image and show the symbol that will be entered).

Is this a bug or it is intentional?

These specific keys provide specific punctuation. I doubt that it’s a bug - as the keys continue to work as intended by the interface (UI) design. The secondary (upper) function always remains accessible using the “downward swipe”.

You’ll note that the two keys that you explicitly highlight do visibly change to their secondary function if you use the Shift-key.
