Unable to Log Into Apple TV

When attempting to sign into my iCloud on Apple TV, I continually get an error that my username or password is incorrect when I know it is being entered correctly. My phone notifies me that there is an attempt to log into my iCloud from another device and gives me the 6-digit code, but there is no where for me to enter the code.

If you see a password box, enter the password (ABC) followed by the code (123) or ABC123 (no spaces).
