If I get the Apple Watch with cellular will I have to pay monthly for it?

It's up to your carrier. They typically charge about $10 a month (in US) additional to your iPhone. Your iPhone and watch MUST use the same carrier. Also see the reply to:

Make sure your carrier supports the watch:

Apple Watch - Carriers - Apple

You don't have to activate the cellular feature if you want to avoid the charge. You will still be able to do everything with the watch as long as your iPhone is nearby.
