“Add Photos Only” not a privacy option for most apps

Under Settings > Privacy > Photos, two of my apps (Wunderlist and DuckDuckGo) give me the options of “Never” or “Add Photos Only.” All the other apps only have the options of “Never” or “Read and Write.” I read that “Add Photos Only” was added as a feature with iOS 11. I’m running 13.5.1. Why is Add Photos Only not an option for all my apps?

When allowing apps to access photos, locations, etc., how much access do they have? Like can apps sneakily upload my all of photos automatically in the background without my user interventions from my iPhone 6+ (iOS v12.4.2)? If so, then is there a way to see what is being transferred and customize their accesses?
