How will apple know I have protection 360 and honor the $29 screen replacement?

You have to use T-Mobiles insurance plan, you should have received materials with your phone when you bought it otherwise call T-Mobile. That is not the same as AppleCare + and you are not eligible for the $29 screen replacement from Apple.
