Why the ECG app still is not available in Russia? Apple promised that the ECG app will be available soon in other countries. Two years passed and the ECG app still not working in Russia. The ECG app was the major reason why I bought Apple Watch series 4 year ago.

That's a question for your country's Medical council or government institution that is in charge if such approvals.
It has nothing to do with Apple at all.

Your countries regulatory council controls what is approved and what is not. Apple cannot turn it on unless it is approved for use there. You should contact your government and ask them.
