After upgrading my XR I have lost ALL personal hotspot options - gone...
I called the ISP and they verified its enabled, rebooted, reset network settings - nothing!
Any ideas?

I had the same problem on my iPad Air 2 and iPad Pro on both iPadOS 14.0 and 14.0.1.
The simple solution was to take out the SIM, wait a few seconds, insert SIM back in, a few seconds more until the network settings were downloaded from the network and that’s it. Personal hotspot is back.

In reply, I found that after this change (and waiting 15 minutes) I could actually swap back to telstra.internet and the hotpot looked to work, but the hotspot wouldn't give out an IP address to use the internet for clients. Using telstra.wap everything works.
I have tried all of the option listed, restored the phone, network reset, sim card, called Telstra etc etc. I have even tried my sim card in another i phone 11 pro with IOS 14.01 and nothing has fixed hotspot, I also can not send or receive any MMS messages even with MMS on and roaming on..... Any other ideas on how to fix this problem... it is a major issue.

I am facing the same problem, but already solved by set APN name, user name and password (ask your operator or googling if you dont know). You can go to menu Setting - Mobile Data - Mobile Data Network - Personal Hotspot section.
I use iphone 11 pro max after update to ios 14.0.1 hotspot is gone...but after i uninstall VPN hotspot is back and work fine
Go to setting>Mobile data>mobile data network and in the personal hotspot section give a user name and return to the settings man screen. Your hotspot option should appear.
