my SSD was cleaned and I try to install new os x with recovery mode but I see this error when I try to Erase my SSD , what is the problems?

The hard drive SATA cable on that model laptop has an extremely high rate of failure especially when using an SSD. You can confirm it by connecting the SSD externally using a USB to SATA Adapter, drive dock, or enclosure as suggested by @Grant.
Also make sure to try erasing the drive a second time immediately after you receive the error. I've seen a problem with Disk Utility where it will fail to erase a drive the first time, but will succeed on a second attempt made right afterwards.

That generally indicates the device or its data cable has failed. Try it in an external enclosure to figure out which.
