Welcome to the Apple Support Communities. We’re glad you decided to reach out about reinstalling the Health app on your iPhone X. If we understand correctly, you’re not finding it on your iPhone and you’d like to know how to get it back.

If you've removed the app from the Home Screen or a folder, the app should still be in your App Library. If you navigate to your App Library, you can add the Health app back to your Home Screen by touching and holding the app, which will appear in the Health & Fitness category.
