iPhone Mail doesn't sync IMAP messages
I have an IMAP Mail account on my Mac and my iPhone. When I read a message on my Mac, it doesn't sync as unread on my iPhone. Why? All systems and apps are up to date.

Sounds like your mail account isn't syncing between devices the way you'd expect them to.
To clarify, does your inbox show that it's fully updated on both devices after reading the email on one of your devices? Also, since syncing is disabled when Low Power Mode is enabled, be sure this feature is turned off on your iPhone to ensure it's syncs correctly.
If you continue to have issues, be certain you're connected to a reliable network. If you're currently experiencing this over Wi-FI, test over cellular.
Lastly, since this is an IMAP account, you can safely remove the account and add it back without losing any emails. To delete it, go to Settings > Mail > Accounts > tap on the account you want to delete and choose "Delete Account".

My inbox was updated, and new mails do appear on both devices.
Low power mode wasn't enabled.
If I can connet to internet on both devices I suppose that rules out network issues.
I deleted and recreated accounts. They seem to be in sync for now.
My mail accounts are out of sync again. I have read messages on my Mac, but they still show up as unread on my iPhone.
Is Mail supposed to support IMAP accounts so that they keep in sync?
If so, how can I set them up so that this issue doesn't occur?

Mail absolutely supports IMAP accounts. This behavior typically occurs when there's a network interruption or an issue with your account details. So those are the best places to look to resolve your issue. Check that both devices have a strong network connection, check to be certain your iPhone isn't in Low Power Mode, try refreshing your inbox by pulling down from the top of the inbox.
With the account being IMAP, the emails aren't downloaded off the server onto your iPhone, therefore it's always safe to delete these accounts and add them back when you experience a problem.
