Macbook dies when I turn it on

Lately, every time I turn my macbook pro on, it starts up correctly but then when I try to do something, like go on the internet, the screen goes black. The trackpad is still clickable. So then I'll press the power button and the icon saying it needs to be connected to power shows on the screen before it fully dies. Once I connect it to power and turn it on, it comes on and works well. The things is, it will be at like 98% battery and still do this every time. Once it's back on I don't need to keep it connected to power for it to continue working. Any ideas on what's going on?

If you hold the Option key and click open the BATT icon in the upper right menu bar what is the Condition(?)


Anything but Normal needs service.
