Health App Steps & Distance is way off the actual

Since updating iOS my health is not counting steps & distance correctly. For example, today I had gone for run and completed around 15.2 KMs and around 15000 steps but my Health app is only showing 12 KM and overall steps to be around 12106 steps which is way off mark.

I’ve checked all settings as mentioned in earlier post on same subject but still the results are incorrect.
