I've Always Used FedEx. But, they include all of that in the box they ship you.
Include some notes, if needed.
If there is anything you need to point out when they receive the item, Include Handwritten Notes with the Packaging

If it’s a fedex return label, you take it to a fedex location.

It'll depend primarily on your location. Don't worry though, they'll send you instructions when you apply for your trade in.
