Headphones not recognized after sleep

This has been driving me crazy.

I have a mid 2012 15" rMBP.

For some time now (can't really remember when I first noticed this), Whenever I wake my mac from sleep, the headphones (whether they were plugged in before or not) are no longer recognised. Sound works fine, I just can't seem to switch from the internal speakers to the headphones (or external speakers).

This does not seem to be a hardware problem, since until the computer goes to sleep the headphones work fine - including after unplugging and plugging etc - and one I reboot the computer switching works fine; until the computer sleeps again.

The only workaround I've found this far is to restart the computer, which is quite annoying. It has to be something in the OS.

I haven't been able to find a solution and I've tried most of what I could find:

  1. resetting NVPRAM and SMC
  2. Erasing the files in ~/Library/Preferences/Audio
  3. Restarting coreaudio daemon
  4. putting the computer to sleep while playing something and waking it up with the headphones, without them and almost any variation of this including the 3 above them
  5. I've reinstalled the computer from scratch (avoided using my time machine backup to prevent re-acquiring the problem), but it's still here.
