I am missing numerous important calls because cannot uncheck Silence while iPhone is locked or Always or anytime actually. And Do Not Disturb is not turned on.

Thats because it’s not under General. It’s at the top level in Settings, 4 down from General.

Those features are only relevant if you have do not disturb turned on, not off.

Have you also checked Silence Unknown Callers in Settings/Phone? Checked the Mute switch above the volume controls? Checked that the volume is up? Checked that Attention Aware is off?
