Passwords appearing in data leak, over and over again?

The first time I got the notification, I rushed to change every password I was notified about. After a few days I was notified that the new password got leaked too. Is my iPhone hacked? What can I do about it?

You mentioned you're receiving a notification about your password. Are you receiving an email? If so, please review the information here: Recognize and avoid phishing messages, phony support calls, and other scams
Are you experiencing these issues with your Apple ID? To help keep your Apple ID secure, use these steps: If you think your Apple ID has been compromised
Do you share your Apple ID or other accounts with someone?
