After entering the only passcode we’ve ever used on this phone, it says verification failed. If we enter a different passcode, it says incorrect passcode.

Two-factor authentication was enabled on the phone.
We were able to get it setup. I don't know they actual issue, but it was resolved after enabling iCloud keychain. Seems a little strange to me, but whatever; it's working now.

Glad to hear you're sorted now. Might be related to an issue with iCloud Account and Sign In that started yesterday and seems to be ongoing: Apple - Support - System Status.
