macbook pro MOJAVE volume setting ALWAYS defaults to LOUD . How stop this?

My macbook pro MOJAVE volume setting ALWAYS starts in LOUD . . . even on reboot, after changing the audio settings. It seems to also default to loud whenever I launch an app.

How can I stop this? I would like to be able to set a default volume that is the starting point for all apps when they open.
