Welcome to Apple Support Communities! We know it can be fun to change your profile picture to different things. We're happy to assist you with this.
Your profile picture that shows in the health app is the same one you use for the profile picture on your contact card. You can change your contact card photo by opening the Contacts app > Tap My Card at the top of your contacts list > tap Edit > then tap Edit under your photo.
