How can I fix this problem on my iPhone 6 Plus?

I was having a problem with my phone before this started first with the screen went all black with a loading screen on it and it loaded all night and basically all day the following day. Then I connect to my iPad so I could call and text until I figured out what to do. Than I took my sim out of my iPhone 6s and put it into my old iphone 5 just to use until I figured out something. But than my iPhone 6 turned on and seemed like it was working fine so I put my sim back into my iPhone 6 Plus and disconnect the iPad from everything at least I pretty sure I disconnect everything from my iPad. And now when I leave the house I loss service. I also can’t call anyone at all besides when I am at home but that’s only on FaceTime and audio FaceTime calls. I can text but that’s only when I am at home.
