How can I keep the display alive on my iPhone while I am telephoning. The problem is that almost immediately Make a call the display blacks out and it is very difficult to get the display back again.

If your proximity sensor is working correctly it will black out or come back on at about 4 inches from your face.
If yours does not behave like that then arrange a repair
