I'm looking to use the apple music family plan for a nursing home community. Is it possible to purchase the family plan and use it on 6 different apple devices without creating 6 different apple ids?

No, not really. in theory you could have 6 devices all signed into one ID except Apple Music will only stream to one device at a time for any particular ID. Plus you'd be skirting along the legal edge of the terms of use of media services.
