iPhone 11 Pro Max, iOS 14.4.1 - Flashlight / Flash randomly turning on

My flashlight keeps randomly turning itself on. Been having this problem ever since I got this phone. It'll even sometimes be facedown sitting at my desk and when I pick it up, my flashlight is inexplicably on, when I know I put it down with it off. After some research, I've gone all the things... turned off tap to wake, turned off raise to wake, completely erased and reinstalled the phone (not a backup/restore), and am running the latest OS.

Most people seem to think it's accidentally hitting the little unremovable flashlight icon on the lock screen, but like I said, I've got my phone completely setup to NOT bring up the lock screen unless I hit the side button, so that isn't the problem.

This is really spooky

To confirm, does this happen randomly or only in certain situations like when the iPhone is in your pocket? Does this continue if you remove all cases or screen protectors from the iPhone?
